Please note that the following items must be referenced on all insurance declaration pages:

1. All persons occupying the apartment, over the age of 18, must be listed on the policy OR they must provide us with separate insurance coverage.
2. List "your apartment community name", along with our address of West Scott Apartments LLC, 211 W Scott St, Chicago, IL 60610 as "Additional Insured" or "Interested Party"..
3. Minimum liability coverage of $100,000 is required.
4. Policy number and effective dates.
5. List your insured address including your apartment street address and unit number.
